To round this off, having updated to a recent unstable MiSTer(2023-08-18 64131f0) I rebound the controller in main and no longer have this problem.
Perhaps something was wrong in the initial binding or something changed but either way working as expected now.

For me it is reproducible from a cold start of the core or from restart. Seen in first level or in attract loop. For most games that don't need this this does nothing, although if you have issues you can toggle the Arcade Card off and hit reset to see if the game returns to normal functionality. Interesting. Thanks for that - this is a really interesting core to see evolve. A good few years ago I played Attack on the Death Star on one of the emus at the time so it was the first thing I wanted to try. I get a lot of glitching with that once it gets to the wireframe intro so looking forward to seeing the improvements.
