Had the same problem, but sound was distorted without too high volume. Updated to the last core version (ao486_20210109), and sound is fine now.

I came to the same problem using default PnP configuration in Win95 OSR. My ao486 version is ao486_20210109 now, but on earlier version (2021 year) situation was the same. Some notes are not playing, some are wrong. In pure MS-DOS 6.22 MIDI is completly prefect.
