Too many other varibles to determine with the little info provided.
How is it connected to the CRT? Scart, Svideo, Composite?
Are you using an analog Io board?
Are you using an active or passive YC encoder board?
Are you using some HDMI to RCA?
What settings do you have enabled in your Mister.ini?
Is it only the sega core that does this?
This sounds like a sync \ resolution change issue.
All good questions, but...
Short of flipping between PAL and another region, once a Genesis cart is running the analog timing is not going to change.
Although some people have reported funkiness with sync-on-green and certain TVs, where the average brightness in certain scenes can destroy your sync.
scart cable on tv and hd on mister, with special cable made for it, i tweaked the ini to work on crt tv, i have very low understanding of crt connections.
I believe its only sega core, probably has something to do with the region, ill change it to EU, also the tv may have issues with region, i had a 15 inch sony and it worked great.
played 1 hour of mortal kombat on a 15 inch and worked great.
Please share the details of your custom cable.

Please confirm you are using an analog io board or some other adapters between the mister and the crt
Also, systems like PlayStation do switch resolutions all the time. Going from in game to a menu to a fmv will do it. Chrono Cross is one of the biggest offenders.
Now genesis may not, I’m also assuming it’s completely random.
If you’re using some video adapter is it powered? Is it getting enough power?
There may be settings in your tweaked ini that only affect the sega core. You can have system specific overrides. But we don’t know because you haven’t shared what your ini contains. So at best we are all going to guess .

Yes I'm speaking only of Genesis. It has a 256 and 320 wide resolution modes, but swapping should be seamless on a CRT, because the vertical refresh does not change.
I guess there is the rare interlaced game like Sonic 2 though.
