So, I set in my ini settings to 1920x1080 60hz and vscale mode to Use integer scale only.
Why is the the option in those cores still greyed?
I have already deleted mister_ini and mister_alt.ini and run ini settings again to generate it, even setting again those 2 options above it is still greyed.
How can I fix this?

I think you need to set ini setting to: vscale mode to: 0 - scale to fit the screen height, instead of to: Use integer scale only.

I tried and still greyed.
Before I had set exactly like in this video https://www.youtube.com/watch?v=HlBSnI3NnfQ
But since it only worked on nes core, I deleted mister ini and mister alt ini, did the changes and see if it worked for the other cores but now all of them are greyed, including nes.
Still no idea how to fix this.

Maybe the core you are trying has an overridden vscale-setting?
Like you are tyring SNES, in your INI ist
vscale=0
but underneath you have specified core-settings like
[snes]
vscale=1

Are you running HDMI alone or running analog out as well?

You should also check any scandoublerfx in the core's menu, that needs to be off for 5x to work, and it's not set in the ini.
