MiSTerCade Fresh Install

I just got a mistercade and trying to get it to work on a Blast City and having a really hard time.
Initially I just put my original MiSTer SD card in but the image is cycling + the buttons don't work - clearly it's misconfigured.
So I am wondering if the best wat yo proceed is a fresh install specific for Mistercade.
I found this:
https://github.com/misteraddons/MiSTercade#readme
but I don't really understand it. Do you need to first run a regular minster install and then mister.ini file? Where? what about the config file? Looks like there are more files on Github?
Sorry all - I'm totally lost

Things seem to work… sort of. Two issues:
1) If you see the attached picture, the screen has a weird ‘concave’ effect. Note that this does not happen with regular boards, so it's not the screen
2) Most importantly, when I launch a rom, it loads but the screen goes dark and nothing happens. I know the rom was loaded bc of I try to load a rom that’s not there I get an error message. But if the rom is there it loads it but nothing shows up.
Any clue?

You need to use the Mistercade ini file.

Make sure you are setting up the CRT screen with the corresponding
mister.ini file to match your Blast City's screen unless it is one of the autoswitching models. The screen will usually need tweaking for the MisterCade, just like you have to tweak when you change individual arcade PCBs.

I used the 15kHz-YOKO ini file. That's the only one I found in the config folder? Maybe that's not right for the Blast City? If not, where do I find the right one or modify it?

Is your 12v solid?
You could try switching the appropriate DIP and switching to 5v? You’d not get sound but it would help troubleshoot.
